It doesn't cost too much to get a decent 5.1 sound setup for your tv, so it's not that rare. Even my little brother saved up enough money to get a great receiver and speakers set.
There's no question that current high-end PCs can play games that would look ten times better than the best console titles, but like you said developers have to make games with all sorts of configurations in mind so that's why the best PC games look about as good as the best console games today. I have the Doom 3 pre-alpha and it looks really good, although it's a lot slower than the final version will be. It's still not lightyears ahead of the best console stuff, but it is better.
And the Quake 3 engine is good for indoor areas but utter shit for outdoor areas.
