Their main concern with the environments has been to make each and every inch of the game beautiful or interesting, not bland like Morrowind. You'll see lots of pretty meadows and forests that reflect the environment of where the developers are located(Vermont, I think).
Create your own monsters? I never heard of that. And aside from the multiplayer and some MS censorship (can't kill kids, take kids on adventures with you, etc.) I can't think of anything major that they took out of the game. When it was first announced they just gave us somewhat vague descriptions of gameplay features which pretty much went wild in our minds because the possibilities are endless when you have no real boundaries, and as the years passed they explained how they would work and things got a bit more realistic.
