The most disturbing story I can remember reading was one I saw on the net four or five years ago, depicting a demented young man drowning an infant in a tub of bleach and describing it in great detail.

The description of the Japanese officer getting skinned alive by a Mongolian soldier in Murakami's The Wind-Up Bird Chronicle was also pretty gruesome.
