Take WoW's quest system and team it up with monthly story updates. Each year could be like a season on TV. A main story arc and a bunch of smaller stand alone stories thrown in to keep things interesting. So each month you would get a series of new quests that furthered the story for that year.
That's not much story, though. Just a little bit to keep things going... I know, it's the way a MMO has to work and it's slightly more "realistic". But it leaves you most of the time just playing so that you can improve your character, and that seems like it'd get old after a while, for sure... yes, some games are trying to deal with that to varying degrees (like Guild Wars, by not being an MMORPG... :)), but on the whole the genre has a formula. And it's not the best formula around for storytelling.

And yes, the next one definitely should have this... but should it have online play as well? And how would that work?
